Financial Information

We deliver the finest care at the most reasonable cost to our patients. Our fees are determined by the complexity of the procedure. They are the same for all patients, whether or not they have insurance. It is our policy that procedures be paid in full at the time of the visit. Please be prepared to pay the day of your appointment. For more expensive procedures, payment plans can be arranged. The annual service charge is 18% (1.5% per month), with a minimum charge of $4.00 per month. For your convenience, we accept Mastercard, Visa, check, and cash. Many times, a simple telephone call can clear up any misunderstandings.

Also, please be aware that any patient under the age of eighteen (18) must be accompanied by the parent or guardian who will accept financial responsibility and be able to sign proper consent forms. In cases of divorce, separation, or legal responsibility, the patient, if of legal age, and/or the parent or guardian accompanying the patient will be ultimately responsible to our office, unless written permission is obtained, before treatment, from the person accepting financial responsibility.

INSURANCE: For your convenience, we will submit our charges to your insurance company. However, we consider the patient, or accompanying parent, responsible for the account. Remember, the insurance contract is between you the insured and the insurance company. We cannot be responsible for any disputes with reimbursements. Also, it is up to you to determine what coverage you have, what percentage of the fee it covers, and what deductible or limit you might have. We do not know the coverage for every policy. Please bring in the appropriate insurance information including the address for claim submission. Most procedures, including wisdom teeth, are covered under dental insurance policies. Only rarely will a medical insurance cover wisdom teeth. Other procedures including biopsies, cysts, fractured bones, accidents, non-dental consultations, etc. are covered under medical policies. Please check with your insurance company specifically to see if any prior approval or pre-treatment authorizations are necessary. Please remember that you are fully responsible for all fees charged by this office regardless of your insurance coverage.
